    The next three describe tooth surfaces, so they are most likely talking about fillings.

    DL=distal lingual (back side and tongue side of tooth)
    DO=distal occlusal (back side and biting surface of tooth)
    MO=mesial occlusal (front side and biting surface of tooth)

    UL=Upper left
    LL=Lower left

    PFM=porcelain fused to metal (metal crown with tooth colored porcelain over the top to look natural)

    Best insurances will cover a lot on cleanings (prophylaxis) or anything preventative and have a smaller deductible.
